I think its important to remind your wife that many women have miscarriages these days. Its not an uncommon occurrence and there is nothing wrong with her, and she is not at fault!

Praying for you both... It is a very difficult thing. My wife has had 2 mis-carriages, one if which was at 19 weeks and had to have the procedure done while I was out of the country. My wife felt the same way blaming herself when it happened, but it isn't there fault at all. Just love on her and comfort her and it will strengthen your marriage in the long run.
